How to Choose a Dental Rubber Dam

These five sheets are not five versions of the same dam. Three are natural-rubber latex (Coltene Hygenic, Coltene Fiesta, and Cranberry Carbon). Two are latex-free (Cranberry SmartDam and the Coltene framed Flexi Dam). The Flexi Dam is also the only one with a frame already attached, and Coltene’s own HySolate guide treats it as a soft-retraction elastomer, not as a heavy latex sheet.

Pick the dam from the allergy history, the retraction you actually need, and whether a frame is already on the tray. A latex-allergic patient does not get “the thinner Hygenic.” A Class II that needs the dam to pull the gingiva back is a poor job for a framed soft dam. Scent is a comfort choice after those two decisions, not before them.

Buying criteria that actually change the procedure

• Latex versus latex-free — natural rubber still retracts harder. Latex-free is mandatory when the patient or anyone on the team has a latex allergy. Do not “solve” that allergy with a scented latex Fiesta dam.
• Gauge — thin latex punches and flosses through contacts more easily and tears sooner. Heavy and extra-heavy latex hold a clamp and retract tissue. Coltene’s latex dams are sold in several gauges; Flexi Dam is a medium elastomer aimed at softer retraction.
• Color and contrast — a dark sheet (Hygenic dark, Cranberry Carbon) makes the tooth and the access easier to see. A pastel scented dam is for the patient who hates the look and smell of plain latex, not for an endodontic access you can barely find.

1. Coltene HYGENIC® Dental Dam

Coltene Hygenic is the plain latex sheet this list is built around. It is natural rubber, sold in more than one gauge (thin through heavy and extra-heavy) and in both 5×5 and 6×6. The 6×6, 36-sheet box is the usual adult operatory pack. Dark colors are the ones that make enamel and a root canal access stand out; light colors are a patient-comfort choice.

Gauge is the clinical decision inside this one brand. Thin is easier to punch and to pass through a tight contact, and it tears when a clamp wing or a floss ligature loads it. Heavy is what you want when the dam itself is helping retract the gingiva and the procedure is long. Coltene still rates latex as the stronger retraction option compared with its own Flexi Dam elastomer.

Who it is for: Offices that isolate in latex every day and will stock two gauges instead of one “medium” box for every procedure.

Tradeoff: Latex allergy ends the conversation, for the patient and for staff who stretch the dam. It does not change clamp selection, frame selection, or the latex question. Cranberry also sells a carbon non-latex sheet under a similar name. If the allergy box on the health history is checked, the latex Carbon dam is the wrong SKU even if the photo looks identical from across the room.

That makes the buying decision small and specific. Choose Fiesta when patients complain about the smell of plain rubber and nobody in the operatory has a latex or fragrance problem. Do not choose it because a chart called it “more comfortable.” Comfort here is scent and color. Retraction still follows gauge.

Who it is for: Pediatric and anxious-adult schedules where the unscented latex dam is the thing patients remember, and latex is still acceptable.

Tradeoff: Fragrance is a second allergy, not a free upgrade. A patient who gags on scented dams should get unscented Hygenic in the same gauge. A latex-allergic patient should get SmartDam or Flexi Dam, not a milder-smelling latex sheet.

Cranberry SmartDam is the latex-free sheet on this list. Current Cranberry pages describe a synthetic, powder-free dam with a high-contrast color (the common spearmint SKU is purple, 6×6, 15 sheets). They publish a low-modulus, high-elongation story — figures in the 600% to 950% range show up depending on which Cranberry or distributor page you read — and a spearmint coating.

Use it when latex is off the table and you still want a separate frame and a sheet you can punch anywhere. It is not the framed Flexi Dam, and it is not a heavy latex retractor. High elongation is useful when you are stretching a non-latex sheet over a clamp; it does not mean the dam will retract gingiva the way extra-heavy Hygenic does.

A framed soft dam is the right first dam for a quick procedure on a latex-allergic patient. It is the wrong dam when you needed a heavy sheet and a winged clamp to hold tissue off a margin.

Who it is for: Operatories that want latex-free isolation without a separate frame, especially for shorter procedures and softer retraction.

Tradeoff: You give up frame choice and you give up heavy tension. The plastic frame is comfortable against the face only if it actually sits there; a twisted frame is worse than a metal one you placed yourself. Do not stock this as “the non-latex Hygenic” and expect extra-heavy behavior.
